We are currently accepting new clients. Prospective clients can download our intake form HERE. Please print it and fill in the required information, to the best of your ability. Not filling out the form in its entirety will delay the review process. We recommend bringing in the completed intake form to our centre. Once submitted, it’ll be reviewed and you will be contacted about the next steps.

In order to ensure your personal information is protected through the internet please only send your intake form through this secure emailing system and not with your regular email.
In the meantime, we encourage you to seek alternative agencies to access healthcare services, including calling 211 for additional support in finding other Community Health Centres.
Appointments for registered clients can be booked by calling clinic reception at 416-593-7655 ext 7. All clients are reminded at time of booking appointments to provide 24 hours notice when cancelling appointments. If you already have your first appointment, you can save time and fill out your registration form ahead of time. Please download our registration form HERE.
Clients wishing to speak to a physician after hours may leave a message with the centre’s phone service and the on-call physician will return the call as soon as possible. In the event of an emergency, clients should report to the nearest hospital emergency room.
Tests results are generally not given over the phone. Clients are encouraged to make an appointment with their provider to privately discuss test results or review information located in their medical records.
Interpretation services are available for clients who cannot speak or understand English. If the client is deaf, American Sign Language (ASL) or other appropriate interpretation can be provided.
